Bolingbrook Chiropractor talks about Back Pain and Exercises you need to NEVER do Again

 My name is Dr. Jeffrey Garofalo and as a Chiropractor in practice for 12 years and having taken care of thousands of spines literally  in Bolingbrook and Surrounding areas I have seen certain exercises that people perform on a routine basis that are damgaing their spines and causing more problems. You will be shocked and quite surprisedto find out that even the exercises that I am about to tell you about are even being taught to you by your personal trainers. 

 The first exercise you should Never perform is pulling or pushing behind your neck. For example if you are trying to work on your back muscles many people will use a machine where they will grab a bar and pull down behind their necks while bending their neck forward. Some people will try to work on their shoulders and grab a bar behind their neck and push up.When you pull or push behind the neck while bending the neck, you are bending the spine forward in the neck level and pushing the discs from teh front of your spine and bulging them out to the back side of the spine. This causes pain first by stretching the discs and second by forcing the discs to push up against the spinal cord. Ultimately either with enough weight or over enough time you can cause a disc herniation. The photo at the left shows the correct way to pull or push which is in front of your neck to keep your neck in a neutral strong position.

The second type of exercise is bending over trying to lift weight off the ground to try to strengthen your low back. These exercises are known as "Dead Lifts" Again when bending forward and putting weight in front of your body you are creating a long lever arm at the lowest level of your spine at L5 and by creating a long lever arm the weight now actually creates more pressure at the front of the disc and again when that happens it forces the disc to bulge backwards stretching the disc creating pain and again with enough pressure or over enough time can cause a disc herniation. In order to strengthen your lower spine you should lie on your stomach                

and arch your upper body and lower body up and down. These exercises are called "Superman" and will be non gravitational and non harmful to your lower back discs.

                                                                                                                                                                                                                   The last exercise that is very dangerous for the spine is putting weight up on your shoulders and performing squats. Again the problem with this is that when you put weight up on your shoulders is that you now become top heavy. Gravity when bending down to squat will pull your upper body forward again creating a long lever arm putting pressure on that L5 disc just like above. I recommend if you want to perform squats you need to take the big rubber exercise ball put your back against it and hold weights in your hand and squat. You now evenly distribbute the weight from front to back and strictly isolate the leg muscles without activating the spinal discs.

Sciatica Pregnancy and a Bolingbrook Chiropractor

  If you are pregnant, have been pregnant, or want to get pregnant you should know that Sciatica or pain from the back down the leg is a common occurence. In today's blog I want to give you what I belieive to be the mechanism for that and give you reasons why you should visit a chiropractor before, during, and after child birth. The first thing that you should know about pregnancy is that you will start to produce a hormone called relaxin. Relaxin is produced so that the ligaments in the pelvis will relax so that the pelvis opens so you can push a baby through it. The challenge is that you are not stationary and as you move and sit and fall and twist you easily start to shift and rotate the pelvis. Typically one side will rotate backwards and one side will rotate forward to match the gait pattern. As this twist starts to happen the pelvis can over rotate now because the ligaments are loose because the body is producing relaxin. When the pelvis over rotates, two tings happen. First increase in inflammation can occur causing chemical irritation to the sciatic nerve casuig pain in the back and down the leg called a lumbar radiculitis and secondly the pelvis bones can physically irritate the sciatic nerve causing the low back pain and pain down the leg. There are many times a woman does not have sciatica during the pregnancy and then after giving birth develops sciatica. Many times a woman delivers their baby and within months after the delivery their relaxin hormone goes away and the ligaments start to tighten. If the pelvis is rotated out of alignment and the ligaments start to tighten they can tighten the pelvis in a malposition sitting very close to the nerve which can trigger a sciatica at any point. 

If is wise for a female to see a chiropractor before getting pregnant to make sure their hips are aligned properly, during pregnancy to keep their hips in alignment and after pregnancy to put their pelvis back in alignment all to prevent the relaxin from allowing the pelvis from becoming a source of Sciatica.

In today's blog post I will be teaching you why some people have really bad neck pain afer and accident and why some don't feel it as bad. A major question I ask a patient after an accident is what position they had their head in while they were being hit. This is a simple question that has some very impactful meanings. Many times we find out that the patient was looking in the rear view mirror looking at the accident about to happen. This puts the neck and heaad in a very weakend position becasue it rotates the neck bones and shortens muscles during impact. Many time a patient will come in with severe neck pain and pain even shooting down their arms. As a result of the patient looking up and turned the force slams the joints together in the neck and furhter rotates the neck. It can also cause a tear in ligaments in the neck more so than if you were looking straight ahead. Turning your neck during an accident is called a risk factor.

A recent study revealed that those who have their head turned during a rear end collision suffer gerater injury than all other collisions. Manohar et al (2006) stated that "In a 5 year retrospective study of 11,000 patients with cervical spine injury, 87.4% had soft tissue injuries.. more than 5 years following whiplash(p.420). The Symptoms commonly encountered included cranial nerve irritation, neck and upper extremity pain, restricted neck motion, and numbness. This study concluded that "head turned rear impact caused significantly greater injury to the cercical spine as compared to head forward rear and frontal impacts" An important part of finding out all the possible problems from a car accident is asking a simple question about head position that has huge impact on your prognsis and final outcome. Pregnancy, Low Back Pain and Chiropractic

It has been this authors personal experience that pregnant woman experiencing low back pain secondary to her pregnancy has been told to “wait and see” over time, with the hope that the back pain would go away. This is predominantly because the “drug option” is off the table with complications to the fetus and most doctors are not willing to take the chance to relieve mechanical (no tumors, fractures or infection) low back pain.  My patients reported to me that their obstetricians told them their pain was a result of altered biomechanics and hormonal changes affecting the muscles and ligament of their spine. Current research has now verified through scientific evidence what practicing chiropractors and their patients have been claiming for decades, that chiropractic work to help relieve pain for pregnant woman with a safe, conservative treatment for both mother and the fetus. As a result of the effectiveness and safety, it now demands that chiropractic be the first referral option for pregnant woman experiencing low back pain.

 

 

According to Petersen, Muhlemann and Humphreys (2014) “Low back and pelvic pain in pregnant women is such a common phenomenon that it is often considered a normal part of the pregnancy [1-3]. However, the high prevalence of this problem (50-80% of women) and the impact that this may have on their quality of life, as well as the fact that back pain during pregnancy is commonly linked to low back pain persisting after pregnancy, mandates that it be taken seriously by health care practitioners. Many of these patients rate their back pain as moderate to severe with a small percentage claiming to be significantly disabled by the pain [6-8]. Pregnancy-related low back pain is most often divided into 3 categories based on location. These are: lumbar spine pain, posterior pelvic pain, or a combination of these two, with posterior pelvic pain reported to be the most common presentation and the location most specific for pregnant patients. Although the etiology of low back pain associated with pregnancy is not definitively known, the predominate theories include biomechanical changes due to the enlarging uterus resulting in an increasing lumbar lordosis and the influence of the hormone relaxin on stabilizing ligaments leading to hypermobility of joints.” [pg. 2]

 

Petersen went on to report “The results of this current study which showed that a high proportion of pregnant patients with LBP undergoing chiropractic treatment reported clinically relevant improvement’ support those published in a recent cohort study as well as the recent randomized clinical trial (RCT) looking at chiropractic treatment for pregnant patients with low back or pelvic pain.” [pg. 5] Meaning, that chiropractic works for low back pain in pregnant woman and it has been proven in many scientific studies. The result showed that at various times during the pregnancy, upwards of 90% of pregnant woman reported positive results. The specific results reported:

 

52% improved at 1 week

70% improved at 1 month

85% improved at 3 months

90% improved at 6 months

 

All of these were with chiropractic care only and no drugs or any other type of intervention beyond patient education by the chiropractor.

 

 

Mullen ET. Al reported that when interviewing midwives, that 88.8% had an experience with chiropractors and 97% was positive. In addition, 94.5% of those had chiropractors treated their children and had a positive experience.  The most revealing statistic is one of safety. 100% of midwives question answered that chiropractic was safe for their pregnant patients.

 

 

We also know that chiropractic is one of the safest treatments currently available in healthcare and when there is a treatment where the potential for benefits far outweighs any risk, it deserves serious consideration.  Whedon, Mackenzie, Phillips, and Lurie (2015) based their study on 6,669,603 subjects after the unqualified subjects had been removed from the study and accounted for 24,068,808 office visits. They concluded, “No mechanism by which SM [spinal manipulation] induces injury into normal healthy tissues has been identified(Whedon et al., 2015, p. 5)

Bolingbrook Chiropractor Dr. Jeff of New Life Chiropractic talks about A Cause of Neck pain

There are many causes of Neck Pain  but today Dr. Jeff a Bolingbrook Chiropractor will discuss one major cause he sees over and over in the office. 

Do you currently have or have you ever had pain in your neck and shoulder that is hard to pinpoint but just aches and throbs and may even cause headaches? Well if you have, you are not the only one! In my clinic, New Life Chiropractic, located in Bolingbrook IL I weekly have this type of condition come in. 

Today's blog is about the casue of that problem known as Facet Capsulitis. Most of us have 24 bones in our spine between the neck, midback, and the low back, and each of those bones connects to the other bones via joints(bone connected to bone). When one of the spinal bones twists turns or bends we call that a Subluxation. Now when the bones move out of its normal alignment, it moves the joint too much or moves the facet joint too much. Protecting that joint is something called a joint capsule which is a ligament type bag that wraps around the joint preventing it from over moving. When the spine rotates or bends past its normal and physiological alignment that bag stretches. When the facet capsule stretches, it then swells and that swelling stretches it even further. The reason you have the diffuse pain you have is because that capsule has nerve endings that feel and when stretched they create a non specific pain in your neck, your shoulder and even perhaps into your head and  your arm.

The pain will persist as long as that capsule remains stretched and irritating the nerves in that capsule surrounding the joint.

Correcting the problem to relieve the pain

Many people at this point will consult their primary care doctor or an orthopedic doctor who will then give them an anti-inflammatory drugs known as an NSAID or non steroidal anti inflammatory drugs for the pain and inflammation. There is a major problem with this because research has shown that taking NSAIDS for ligament inflammation actually casues long term ligament damage(1.)

The natural way we correct this facet capsulitis is to realign the joint through a chiropractic adjustment thereby taking the tension off the capsule and then having our patients use ice allowing the inflammation to calm down naturally so as to heal the ligament capsule properly for the long haul.

Disc Herniation Sciatica and a Bolingbrook Chiropractor

Many patients come to my office with disc hernitations in their lower back with accompanying sciatica. In today's blog I would like to describe first what a disc herniation is and then describe what sciatica is and  give you the best possible natural solution to solve both conditions.

A disc herniation is when the cushion inbetween the bones in the lower back collapses under pressure either long term or a trauma like a car accident or fall. The middle of the disc is like a jelly bean and the outside of the disc is made up of concentric cartilage circles of water. A hernaition occurs when the middle jelly bean tears through the outer layers and the material leaks into the area where the nerves or spinal cord are. A herniation can occur over time as the disc becomes weak and then a tear occurs or if a linear tear is present an acute injury is the cause. Either way when that material leaks out it is toxic and cuases chemical irritation and physical pressure upon the sciatic nerve which can cause pain down the leg or into the buttocks or into the groin area. The Sciatic nerve is the largest nerve in the body and made up of several nerves that come out of your lower back and bind together and run down the front and back of your legs. A reasearch approach to Disc Herniations and Sciatica and Bolingbrook Chiropractic

One of the best ways to resolve disc herniations non surgically is with a chiropractic adjsutment as opposed to surgery called a micro discectomy.

It was reported by McMorland, Suter, Casha, du Plessis, and Hurlbert in 2010 that over 250,000 patients a year undergo elective lumbar discectomy (spinal surgery) for the treatment of low back disc issues in the United States. The researchers did a comparative randomized clinical study comparing spinal microdiscectomy (surgery) performed by neurosurgeons to non-operative manipulative treatments (chiropractic adjustments) performed by chiropractors. The above study indicates that a conservative non-operative approach of chiropractic care prevented 60% from needless surgery. 

it was concluded that chiropractic is a viable, first line treatment option.

These studies along with many others conclude that a drug-free approach of chiropractic care is one of the best solutions for patients with surgical lumbar discs and sciatic pain.

Bolingbrook Chiropractor Dr. Jeff talks about the sources of Pain after a Car Accident

Dr Garofalo, Bolingbrook ChiropractorThere are many sources of pain after a car accident and in order for you to have the best possible outcome your doctor needs to be trained in the specialty of Trauma. Most family doctors and even immediate care doctors are not fully trained to unerstand why you are having the pain you are having. I, Dr Jeff Garofalo am a trauma trained specialist and in today's blog I would like to help you understand where your pain could be coming from as there are multiple areas and it might take days to weeks for all damaged tissue to show itself.

At New Life Chiropractic a chiropractic office in Bolingbrook, IL, we have an onsite x-ray machine because the first thing we want to do after an accident is take some very special x-rays called a DAVIS series. This series of x-ray is actually 5 x-rays of your neck to make sure that you don't have any fractures in your neck, and secondly it helps me determine if you have potential Ligament damage in your neck a huge source of pain and an inury that will never go away and therefore permanent causing premature spinal arthritis. The next generator of pain we assess for is muscle spasm, you know those very tight muscles you might be feeling in your neck, shoulders, or in your lower back. At New Life Chiropractic we use a technique called palpation meaning I use my hands to feel those muscles and let you know whether they are in mild, moderate,or severe spasm as a result of your accident. Muscles usually go into spasm secondarily and as a protective mechanism becasue something else in the spine is damaged. I will inform you of which category you are in.

We then assess for possible disc damage by performing several in office tests called orthopedic tests putiing your spine in certain angles that could bring out possible disc injury and back these tests up with a neurological exam and finally if I think a disc has been herniated or an already  damaged disc has been aggravated I will send you for a very UNIQUE MRI. Unique meaning it is not like other MRI companies perform. I order MRI with very very thin slicing so as not to miss any potential damage because I wnat to make a proper diagnosis, prognosis and give you the very best care. At this point once I have all the data if I feel this case is one that I can accept we will start you on your road to recovery with chirorpactic and spinal rehab. Low Back Pain Golf and a Bolingbrook Chiropractor

As a Chiropractor in Bolingbrook, Il and I am privileged to be surrounded by some pretty great golf courses like The Links at Carillon. This also means that I see alot of golfer's with low back pain in my Bolingbrook Chiropractic office. Today I would like to give you a few tips from my Golf Clinic I am presenting this summer with PGA Pro Chris Dimmitt. 

     First let's discuss the cause of lower back pain:
1. Overuse in one direction of your spine. If you are a righty or lefty think about how many times you will swing that way for your entire golf career. Now think about rotating the joints in your lower back that way for all those swings and now you start to see how you can wear and tear those joints on that side of the spine leading to back pain. 

2. Inflammation as you constatnly rotate the spine to that side of the spine will casue pain after the round along with soreness and stiffness the following days.

Tips to prevent Low Back Pain

1. To prevent one sided shortening of muscles and over rotating at every tee box I want you to take 5 very slow and gentle practice swings in the opposite direction that you normally swing. This will create balance in your swing preventing wear and tear preventing low back pain.  

2. On the way to the course get an ice pack and put it on your lower back to decrease your core body temperature thereby lowering inflammation decreasing pain you will experience right after and days later.

Bolingbrook Headache Relief Dr. Jeff Garofalo

Headaches are common but not normal. Many people get headaches but they are a sign that something is not correct in the body except for when you are cleansing. A cleansing headache usually comes from either detoxing through nutriton, or giving up caffeine and sugar. Those are not the headaches we are discussing today.

I am a Bolingbrook Chiropractor and see many patients who come with headaches, 90% of which are tired of medicating. Many of the headache sufferer's who come for help at my Bolingbrook clinic have something in common. Many of their headaches come from their necks. Many have alot of muscle tension that is secondary to their spinal bones in the neck being shifted out of alignment and pinching the nerves which restrict the proper blood supply to the head causing tight muscles, knots and headaches.

The Merritt Herald article also reported that this information is not new, noting that, "In 1995, a team of MDs at Syracuse University established neck problems as the cause of many headaches with scientific, anatomical proof." The researchers were headed up by Dr. Rothbart, a medical doctor.

In an interview after the research, Dr. Rothbart stated, "Some brilliant people have put their hearts, souls and minds to this (headache) problem and haven't come up with anything. All we've been able to do is treat people with an array of medicines, one after the other, and hope the side effects won't be too bad. We couldn’t believe it at first. We've been able to put together a scientific explanation for how neck structure causes headaches — not all headaches, but a significant number of them. It's true that chiropractors have been saying this for years. Unfortunately, many (medical) doctors tend to have a jaundiced view of chiropractors, but they were right about headaches."

As a chiropractor in Bolingbrook the main tool I use to naturally help those with headaches is the chiropractic adjusment which has been very successful.

The American Chiropractic Association release reported on research of how chiropractic helps headaches by noting, "A report released in 2001 by researchers at the Duke University Evidence-Based Practice Center in Durham, N.C., found that spinal manipulation (adjustments) resulted in almost immediate improvement for those headaches that originate in the neck, and had significantly fewer side effects and longer-lasting relief of tension-type headache than a commonly prescribed medication."

 My name is Dr. Jeff Garofalo and "Learning never Stops" is my Philosophy.

Therefore every year I strive to learn something new as I am on my journey to become a master chiropractor. In the past year I discovered that many more people seek out the help of a chiropractor for auto accidents and whiplash injuries than I had perviously thought. As a Chiropractor in Bolingbrook I was shocked to see how many accidents occur in the town on a daily basis. I decided that I had to learn more in order to help my community. I started out on my year long post doctorate journey studying not only at a Seminar in Las Vegas and St. Louis but I also studied distance learning via the internet while my wife and kids went to bed. I learned in depth how to take the correct history and perform an in depth examination looking for injuries that most doctors aren't aware of that can answer many patient's questions as to why they are feeling what they are feeling. I learned how to read an MRI and put together a team of fantastic doctors to help me get the best results for my patients. This group includes a neurosurgeon, nuerologist and even a pain management doctor. I've learned how to diagnose disc herniations and damage to ligaments that most other doctors are not doing. I learned how to document and report properly so I can communicate with an attorney properly if one is used. As a reslut of my studies my Bolingbrook patients are healing quicker and having all injuries discovered and documented properly.
